Remote Patient Monitoring (RPM) Privacy Policy

Our Remote Patient Monitoring (RPM) program is designed to support your ongoing care through the secure collection and review of health information. This Privacy Policy explains what information we collect, how it is used, how it is protected under HIPAA, and with whom it may be shared.

As part of the RPM program, we may collect the following information:

  • Your name, date of birth, and contact information
  • Health data generated from RPM devices (such as blood pressure readings or other physiologic measurements)
  • Health-related responses you provide during RPM communications
  • Information necessary for care coordination and program participation 

We only collect information that is relevant to your care and participation in RPM services.

How We Use Your Information

Your information is used solely to:

  • Monitor your health status remotely
  • Identify trends or potential concerns early
  • Support clinical decision-making
  • Coordinate care and escalate concerns when medically necessary
  • Facilitate RPM services and required insurance billing

RPM services do not replace emergency medical care.

We do not sell, rent, or share your information for marketing purposes.

Your information is shared only when necessary, and limited to:

  • Your assigned virtual physician or licensed medical provider for clinical review and escalation of care
  • Our billing company, strictly for insurance billing, claims submission,     and payment processing related to RPM services

We do not share SMS or RPM opt-in/consent information with third parties for any purpose.

HIPAA Compliance and Data Protection

We comply with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state and federal privacy laws. Safeguards are in place to protect your information, including:

  • Secure systems and restricted access
  • HIPAA-compliant vendors and business associate agreements (BAAs)
  • Access limited to authorized personnel involved in your care or billing

Your Rights and Choices

You have the right to:

  • Access or request corrections to your health information
  • Withdraw from the RPM program at any time
  • Opt out of RPM communications if you choose

Participation in RPM is voluntary and based on your consent.
